Search for Existing Business/Facility Records

If the facility holds an active registration or has ever held a registration with the Oregon Board of Pharmacy and you do not have the eGov Registration Code, contact the Board at pharmacy.licensing@bop.oregon.gov to obtain your eGov Registration Code and renewal ID. Do not apply for a new license.

  • The registration code will only be released to an authorized agent of the facility.
  • To access a renewal, you must create a username and password and successfully link it to the facility registration record.
  • If you do not know the facility license/registration number use Verify a License to look up your Oregon Board of Pharmacy license/registration number.
  • Facilities can renew an existing facility license/registration, or order a certified copy of a registration at this time.
  • Please complete two of the fields below and click “Search” to proceed with your registration.
  • Contact the Board at pharmacy.licensing@bop.oregon.gov if you need to request a renewal ID or eGov registration code for an existing license/registration.

    New applicants:

    Online applications are available for Manufacturer, Controlled Substance, Nonprescription Drug Outlet - Class A (over-the-counter) and Nonprescription Drug Outlet - Class C (Medical Device, Equipment and Gas) registrations. We hope to have additional online applications and services for additional registration types in the future.

  • To create an account and apply for a new license/registration online, complete a “search” below.
  • Please use any digit such as “1”, or “0” or combination of digits to complete a search.
  • As a new user, your information will not be found. You will then be taken to the following page to create a new account and apply for a new license. Please complete all sections of the registration page.
  • If you currently hold, or have ever held a registration for this facility, please contact the Board for assistance in locating your account. DO NOT apply for a new license. Doing so may result in an overpayment of fees. NOTE: All fees are nonrefundable.

    Facilities that are logging in to pay an invoice must enter below the unique Invoice PIN that was sent by the Oregon Board of Pharmacy.
